Over network layout

 Over network layout
Hi,
I'm interested in supplying high-speed internet to MDUs and I'm interested to know what the best network layout would be.
Here is the design I'm assuming from the top of the stream:
T1/T3fromISP<--->Router<--->CMTS<--->CableModems

I have a few questions in regards to this:
Is this the most efficient layout?
Have I missed any devices (other than DHCP/TFTP/TOD Servers)?
Do I need a Headend?
